to make many difficult decisions regarding medical and financial options as well as emotional and social factors.

Therefore, prior to receiving treatment for infertility, you will have a financial consultation with one of our patient account representatives to assist you in determining your benefits, identify non-covered services
and assess your role and responsibility in assuring that MMC receives payment in a timely manner.

Insurance
The physicians and laboratories of Genesis Fertility & Reproductive Medicine, a Division of Reproductive Endocrinology and Infertility at Maimonides Medical Center, participate with the following insurance carriers:

Referrals
The patient is responsible for obtaining a valid referral when scheduling an appointment with any of our physicians or laboratories. Present the referral to either the front desk or the billing department when you arrive for your appointment. Depending on the service(s) performed, some insurance carriers will require more than one co-pay per visit. If you are already an established patient, be sure to keep your referral current.

The IntegraMed® Shared Risk® Refund Program

At Genesis we offer a wide range of infertility treatments in our state-of-the-art facility. We understand that a patient’s decision to pursue advanced infertility treatment can be stressful, especially when there is limited insurance coverage for the required treatment. In response to these financial concerns, Genesis has partnered with IntegraMed® to offer our patients some unique financing options to help paying for treatment more manageable. Genesis is the only practice in Brooklyn and Staten Island credentialed and approved by IntegraMed to participate in its national network of infertility specialists.

Genesis offers IntegraMed’s Shared Risk Refund Program® as well as treatment financing as alternative financial options through which patients can pursue their dreams of becoming parents. These programs allow patients to determine the cost of treatment in advance, taking away the anxiety of facing unforeseen financial concerns related to their enrollment in advanced treatment programs such as in vitro fertilization (IVF).

Patients who qualify for this program pay IntegraMed a flat fee for up to three fresh IVF cycles and three frozen embryo transfers. The primary benefit of this program is that it increases the patient’s chance of success with IVF by allowing her to attempt up to six treatment cycles for one fixed cost. In addition, it protects the patient from the financial loss associated with failed treatment. When the patient takes a baby home from the hospital, IntegraMed has earned its fee. If, however, the patient completes the program and does not take a baby home, a refund (70% for IVF and 100% for donor egg treatment) is given, preserving resources for the patient to pursue other family building options.
